We understand that referring your patients to another provider requires a significant amount of trust. As a referring GP, you need to know that your patients are receiving safe, appropriate care that aligns with your overall management plan.


We are deeply committed to a multidisciplinary approach, acting as a reliable bridge between patient behaviour and your GP-led Chronic Conditions Management Plans (GPCCMP).

Our Clinical Approach: The Health Blueprint™

We do not rely on ad-hoc or purely symptom-based treatments. Every patient referred to our clinic undergoes our proprietary Health Blueprint™ assessment. This ensures we establish objective baseline data before any care commences.


Our assessment protocols include:

• Digital Posture Screening: To visually quantify structural imbalances.

• Comprehensive Neurological Examinations: To assess nerve function and identify potential red flags.

• Standardised Functional Scoring: To track pain levels and mobility improvements over time.

• Pre- and Post-Care Structural X-rays (when clinically indicated): To provide objective visual evidence of structural changes.

Seamless Communication & Reporting

We know that solo and small-practice GPs are incredibly time-poor. Our goal is to support your care plans without adding to your administrative burden. When you refer a patient to Adjusting to Health, our dedicated Care Coordinator ensures you are kept in the loop.


We provide:


•Prompt Acknowledgement: Confirmation when your patient commences their initial 10-week care phase.

•Objective Progress Reports: Concise updates detailing changes in their functional scoring and posture.

•Discharge Summaries: Clear documentation upon completion of their care plan or if they are referred back to you for further medical management.

Scope of Practice & Referrals

Care is only accepted when clinically appropriate. If a patient presents with conditions outside our scope of practice, or if they require medical intervention, we will promptly refer them back to your care.

We accept referrals under the Medicare Chronic Disease Management (CDM) framework.

As we are a premium provider, patients will be required to pay a small gap fee.
